Trauma can leave the brain treating certain memories and cues as if they remain present threats. Research suggests ketamine may influence the processes by which fear memories are stored, reactivated, and updated — potentially supporting emotional learning during and after treatment. The concept of memory reconsolidation offers a framework for understanding how rigid emotional responses might become more flexible. Whether ketamine meaningfully supports reconsolidation-based therapeutic work in clinical settings remains an open question.

What is reasonably well supported

• Memory reconsolidation — the updating of a stored memory when it is reactivated — is a well-established phenomenon in neuroscience. • Ketamine affects glutamate and NMDA-dependent signaling central to memory formation and updating. • Preclinical studies show ketamine can influence fear memory processes, including reconsolidation and extinction. • Some clinical observations describe patients processing traumatic material with reduced overwhelm during or after ketamine.

What remains uncertain

• Whether ketamine meaningfully enhances fear memory reconsolidation in human therapeutic settings. • How the timing of trauma-focused psychotherapy relative to ketamine dosing affects outcomes. • Why some patients experience emotional relief while others feel more vulnerable or destabilized. • Whether neuroimaging markers can predict which trauma patients are most likely to benefit.

Clinical framing

Some patients describe traumatic memories feeling less charged or more integrated after ketamine-assisted work, while others feel more exposed or raw. These responses vary and are not predictable. Ketamine should be framed as potentially supporting the conditions for emotional learning — not as "erasing," "healing," or "processing" trauma automatically. Trauma-focused integration and careful pacing remain essential.
