For Referring Clinicians

This practice collaborates with therapists and referring clinicians for carefully screened ketamine-assisted psychotherapy (KAP) referrals in Tel Aviv — including select patients visiting Israel temporarily.

Who This Page Is For

Psychotherapists and counselors
Psychologists
Psychiatrists and psychiatric nurse practitioners
Primary care physicians and other medical referrers
Integration therapists and trauma specialists
Clinicians outside Israel whose patients are visiting temporarily and may need continuity, a booster session, or short-stay KAP support

Patients Who May Be a Clinical Fit

The following profiles are often discussed in the context of KAP referrals. Final candidacy is always determined through a comprehensive psychiatric evaluation, medical history review, and safety screening.

Treatment-resistant depression with inadequate response to two or more adequate antidepressant trials

PTSD or complex trauma remaining symptomatic despite ongoing psychotherapy and/or pharmacotherapy

Persistent depressive episodes with limited conventional treatment response

Patients already engaged in psychotherapy who may benefit from a structured KAP model to deepen psychological work

Motivated patients seeking to combine pharmacological and psychotherapeutic approaches in a supervised clinical setting

Select visiting patients seeking short-term continuity or clinically appropriate booster support while in Israel

All referrals remain subject to full psychiatric evaluation, medical and psychiatric appropriateness screening, and clinical safety review. Referral does not guarantee treatment.

The Psychiatrist-Led KAP Model

This practice offers two distinct referral pathways: psychiatrist-led ketamine-assisted psychotherapy (KAP), and limited-scope psychiatric consultation or second opinion for a defined clinical question. The KAP treatment model includes:

1

Psychiatric screening and evaluation

A structured clinical assessment covering diagnosis, medical history, psychiatric history, contraindications, and treatment goals.

2

Preparation

Pre-session preparation work with the patient to establish intentions, address concerns, and ensure clinical readiness.

3

Ketamine-assisted psychotherapy sessions

Supervised sessions integrating the pharmacological effects of ketamine with structured psychological support. Psychiatric consultations are typically conducted online; treatment sessions are arranged individually and may be conducted in the patient's home when clinically appropriate.

4

Integration and clinical follow-up planning

Post-session integration is an essential component. Ongoing integration work may be conducted by the patient's primary therapist where applicable.

5

Collaboration with the referring clinician

Where the patient consents, communication with the outside therapist or prescriber can be arranged, including referral updates and treatment summaries.

Psychiatric Consultation & Second Opinion Referrals

In addition to KAP, the practice offers a limited-scope psychiatric consultation or second opinion referral pathway. This is a focused consultation for a defined psychiatric question — not ongoing psychiatric management or routine medication management.

  • Focused consultation for a defined clinical question
  • Secure video consultation
  • Written psychiatric opinion (target: 5 business days after consultation and receipt of required information)
  • One limited clarification interaction included
  • Treating-clinician handoff for medication recommendations
  • Not emergency care · Not ongoing management · No prescribing or refills in the initial model

Medication-focused consultations ordinarily require an identifiable treating or prescribing clinician who can independently evaluate recommendations. This pathway is separate from KAP and does not imply KAP eligibility.

How Collaboration Works

The practice genuinely values communication with treating clinicians and welcomes thoughtful referrals from colleagues who know their patients well.

How Collaboration Works

  1. Referral or patient inquiry
  2. Free 15–20 minute screening call
  3. Secure intake forms and psychiatric evaluation if appropriate
  4. Medically supervised in-home IM ketamine session when clinically appropriate
  5. With patient consent, concise coordination summary for the outside therapist

Collaboration requires patient consent and does not replace independent clinical judgment by either clinician.

  • Outside therapists are welcome to remain actively involved in the patient's care
  • Integration work following ketamine sessions may continue with you as the patient's primary therapist
  • The practice can coordinate with the referring clinician when the patient provides appropriate consent
  • Referral updates and brief treatment summaries can be shared when clinically appropriate and authorized
  • The goal is collaborative care — not replacing the patient's existing therapeutic relationship unless explicitly arranged
  • The psychiatrist role focuses on medical evaluation, session oversight, and clinical safety — not ongoing psychotherapy

What the Practice Can Provide

  • Psychiatric evaluation for KAP candidacy
  • Medical oversight and medication review
  • In-home IM ketamine session for appropriate patients
  • Written aftercare and safety guidance
  • Concise coordination summaries with consent
  • Option for preparation/integration support when clinically needed

What Remains with the Outside Therapist

  • Ongoing therapeutic relationship
  • Deeper preparation and integration when appropriate
  • Longitudinal psychotherapy goals
  • Ongoing crisis planning if relevant to their scope and relationship

The practice does not provide emergency or crisis care through referral forms, the website, or the patient portal. Urgent safety concerns should be directed to emergency services or the appropriate crisis pathway.

Referrals for Patients Visiting Israel

The practice may be able to assist with select referrals for patients who will be temporarily in Israel and have an established clinical relationship with a referring clinician abroad.

Short-stay continuity planning for patients already in a KAP or psychotherapy program

Booster sessions when clinically appropriate and prior ketamine history is documented

Coordination with an existing outside prescriber or therapist when documentation is adequate and timing permits

What Helps to Send in Advance

  • Diagnosis and brief clinical summary
  • Current medications (including dose and prescriber)
  • Prior ketamine history, if relevant (agent, route, response)
  • Any recent medical or psychiatric concerns
  • Reason for referral and timing constraints
  • Your contact information as the referring clinician

Due to the clinical complexity of short-stay cases, these referrals are evaluated individually and not all can be accommodated.

What Information Helps With a Referral

For standard referrals, the following information is helpful at the outset — no formal referral letter is required to make initial contact:

  • Patient name and best contact information
  • Reason for referral
  • Current diagnosis and primary symptoms
  • Current medications
  • Relevant psychiatric and medical history
  • Prior ketamine response, if any
  • Your name and contact information as the referring clinician

Frequently Asked Questions — For Clinicians

Privacy and Secure Communication

Mindful Pathways uses a low-friction public screening request for initial contact and a secure patient portal for detailed clinical information. This helps reduce barriers while keeping sensitive information in the appropriate setting.

What belongs where

Privacy reminder

Please do not include detailed psychiatric history, medication lists, trauma details, suicidal thoughts, or other sensitive clinical information in this public form. If screening suggests evaluation may be appropriate, detailed information will be reviewed through secure intake.

Emergency boundary

Public forms, email, WhatsApp, AI tools, and the patient portal are not emergency services. If you are in immediate danger or may harm yourself or someone else, contact local emergency services or go to the nearest emergency room.

Discuss a Referral

If you have a patient you believe may benefit from a KAP evaluation — or if you would like to discuss a visiting patient case — please contact the practice directly. The practice welcomes direct communication with referring clinicians.
