Overview: It's More Than One Appointment
A ketamine therapy session is not simply a medication appointment. It is a carefully structured therapeutic process spanning several days — preparation before, the dosing session itself, and integration work afterward. Each phase is clinically essential.
Preparation Sessions
Before any ketamine session, patients complete one or two preparation therapy sessions with Dr. Rosenbach via Zoom. These sessions cover: your intentions and goals for the experience, any fears or concerns, psychological grounding techniques, what the altered state feels like and how to work with it productively, and practical instructions (fasting, medications to hold, companion requirements).
Preparation is not a formality. Research in psychedelic-assisted therapy consistently shows that 'set' (mindset and psychological readiness) is among the strongest predictors of therapeutic outcome. Patients who are well-prepared navigate the experience more effectively and extract more lasting benefit.
Session Day: Set-Up
Dr. Rosenbach comes to your home. Upon arrival, he takes your vital signs and conducts a brief check-in about your current emotional state, any concerns, and your intention for today's session. You make yourself comfortable — lying on your bed or sofa, with eye shades and headphones. The space is kept quiet, calm, and private.
Dosing: IM Administration
For intramuscular (IM) ketamine, Dr. Rosenbach administers a precisely calculated dose by injection into the deltoid muscle. Effects begin within 5–10 minutes. Within 15–20 minutes, the full altered state is established. Vital signs are monitored throughout. You remain conscious throughout — this is not general anesthesia.
The Ketamine Experience
The altered state typically lasts 45–90 minutes. Curated music plays through your headphones — carefully selected instrumental, ambient, or classical compositions chosen to support deep introspection. Music is not incidental: research shows it profoundly shapes the emotional arc of the experience.
Common experiences include: a sense of floating or weightlessness, visual patterns or abstract imagery, deep emotional processing, altered sense of time, ego dissolution (a temporary reduction in the usual sense of 'I'), and a feeling of expanded perspective. Some experiences are profoundly peaceful; others involve difficult emotions that arise and pass. Dr. Rosenbach is present throughout.
The intensity of the experience is not the measure of therapeutic value. Some patients have subtle, quiet sessions and benefit significantly. The preparation and integration work is where much of the lasting therapeutic value is created.
Re-Orientation
As ketamine wears off over 1–2 hours, you gradually return to ordinary awareness. Dr. Rosenbach gently checks in, helps orient you, and briefly explores initial impressions. You must not drive on the day of your session. Your adult companion remains with you. Most patients feel calm, emotionally open, and gently tired.
Integration Session
Within 24–72 hours — while the experience is still vivid and the brain remains in an enhanced neuroplasticity state — you meet with Dr. Rosenbach via Zoom for an integration session. Together you explore what emerged: emotions, images, insights, or difficult material. The goal is to make meaning from the experience and identify concrete applications to daily life.
Without integration, the benefits of even a powerful ketamine session can fade within days or weeks. Integration therapy transforms a psychedelic experience into lasting psychiatric change. This is what distinguishes KAP from ketamine infusion clinics.
