International Continuity Ketamine Services
For Visiting Patients with Established Ketamine Treatment
I offer a limited, psychiatrist-led continuity-of-care ketamine booster service for patients who are already receiving ketamine treatment under the care of a licensed provider outside Israel and who are temporarily visiting Israel.
This referral pathway is appropriate for patients who:
- Are currently receiving, or have recently received, ketamine treatment
- Have an established treating provider abroad
- Are visiting Israel short-term (travel, family visit, work stay)
- Require a booster session to maintain treatment gains
- Can provide clear medical documentation from their home clinician
My role in these cases is intentionally narrow and conservative.
I provide:
- Review of prior treatment documentation
- Safety assessment for short-term continuation
- Psychiatrist-administered IM ketamine
- Medical monitoring during the session
- Brief integration and aftercare guidance
- Summary communication back to the referring clinician when appropriate
I do not assume:
- Long-term treatment responsibility
- Diagnostic reevaluation unless clinically indicated
- Medication management outside the ketamine session
- Ongoing psychotherapy unless separately arranged
Care returns fully to the referring provider after the patient's visit.
International continuity-of-care work differs from clinic-based ketamine treatment in several important ways:
- There is no standing clinic infrastructure supporting follow-up
- Medico-legal responsibility rests entirely with the treating psychiatrist
- Dosing decisions must be made based on external records
- Sessions are time-limited rather than program-based
Clear documentation protects the patient, the referring clinician, and me. To proceed safely, the following information is required prior to scheduling:

Confirmation of prior ketamine treatment

- •Route(s) used (IV, IM, SL, IN)
- •Typical dose range
- •Date of most recent session
Clinical indication
- •Primary diagnosis (e.g., TRD, PTSD)
- •Brief treatment rationale
Safety history
- •Any adverse reactions
- •History of psychosis, mania, or dissociation beyond expected range
- •Cardiovascular or medical contraindications
Current medication list
- •Including psychiatric and cardiovascular medications
Helpful (But Not Always Required)
- Treatment summaries or discharge notes
- Recent psychiatric assessment
- Consent forms used by the home clinic
- Contact details for the referring clinician
If records are partial or unclear, a formal record review may be required before approval, additional clarification may be requested, or in some cases, the service may not be offered. This is not punitive — it is a safety boundary.

Required for all new visiting patients. May be waived for complete documentation or direct clinician referrals.
